Requirement for Crew Visa

1. Original Passport with at least six months validity and available visa pages.
2. Duly filled one Crew Visa application form with two photos taken within the past six months. The visa form and work history form must be filled completely.
3. For Crew members from air line:
   a. The Invitation Letter by a local air line/ recommendation for employment as current crew member.
   b. The Permission/ Recommendation by the Ministry concerned recognizing the airline.
4. For Crew on a special flight: 
   a. The Recommendation by the country concerned to certify that the special aircraft is used for the official visit by the Hade of State or Government.
   b. The Landing Permission by the Ministry concerned and application by Sponsor Company for the crew and personal undertaking in the event of a special aircraft owned by private individuals.
5. A Copy of Crew License.
6. Payment can be payable to “Consulate-General of Myanmar” in Cashier's Check or money order only. Payment arranged through Cash/ Credit Card/ Debit Card/ Personal Check is not accepted.
7. Prepaid Self-Address Return Envelope.
   (For those who apply through courier services can use FedEx, UPS or USPS with Tracking Number. Consulate-General of Myanmar assumes not responsible for any delay or loss of mail or documents while under the custody of courier services. The applicants shall not reuse the tracking numbers of all used and submitted envelopes.)
8. Incomplete applications will be rejected at the counter. Incomplete applications through courier services will be sent back to its original destination.

 
Visa Fees

Visa Information

• The stay for the Crew Visa is 90 days, which is entitled to extend as per requirement.
• The validity of the Crew Visa (Single Entry) is 3 months from the date of issue, which cannot be renewed or refunded. The Consulate will issue visa for completed application as soon as receive it.
• The Consulate will not take any responsible for too early and too late applications.

Photography Guide

• The photograph must have been taken within the last six months
• The photograph should be taken in color and must be with a white.
• Side or angled views are Not accepted.
• Photo Size: 35 mm x 45 mm or standard photo of 2 in x 2 in
• Photo Appearance: The photograph must be a full-face view in which the visa applicant is facing the camera directly.
• Digital Photos: Digitally reproduced photographs must be reproduced without discernible pixels or dot patterns.
• Photocopied photographs are NOT accepted.
